某個下雨的日子，經營寵物醫院的張文浩（李善均 飾）帶著還有一個月就要與之共赴婚姻殿堂的薑善瑛（金敏喜 飾）前往老家看望父母。途中文浩下車購物，而善瑛在接了一通電話之後便從男友身邊神秘失蹤。焦急的文浩四處尋找，結果卻發現一樁樁令他難以置信的事實。善瑛曾於2007年債台高築宣布破產，而當年的照片與他失蹤的未婚妻竟完全是兩個人。這個失蹤的女人真身為何？她的背後又隱藏怎樣的秘密和故事呢？ 本片根據日本作家宮部美雪的同名原著改編，“火車”來自佛教用語，指載著作惡亡靈前往地獄的冒火的車子。《觀無量壽經》雲：“人以惡應墮惡道，命欲終時，地獄眾火俱至，必有火車來迎。”
